Is this something I should do every year? Or only when it seems like there's trouble?

Depends on what you burn.

If you have a wood fireplace and never burn anything but very well seasoned hardwood and keep the fire hot all the time, you may be able to get away with it slightly less often.

If you burn wood and are like most folks, though, you should get it cleaned every year. Our chimney in this house was so bad, we had no choice but to have it lined (or torn down, but that would be too expensive) and then replace the wood stove with a pellet stove. The creosote in the chimeny was thick and glassy, and was just asking for a fire.

I think once a year is the safe bet. There are also logs you can get that when they burn they release something that helps to cut down on the "gunk" that builds up in the chimney. I don't think they are meant to replace regular cleanings but they don't hurt to burn every now and again to help keep things clean.
